General Information

The ADHD Symptom Checklist-4 (ADHD-SC4) is a 50-item rating scale completed by parents (information pertaining to the parent checklist can be found below) or teachers to screen for AD/HD and oppositional defiant disorder (ODD). The ADHD-SC4 also contains the Peer Conflict scale which measures peer aggression and the Stimulant Side Effects Checklist which monitors medication.
