ADHD Symptom Checklist - 4 - Teacher Checklist
General Information
The ADHD Symptom Checklist-4 (ADHD-SC4) is a 50-item rating scale completed by parents or teachers (information pertaining to the teacher checklist can be found below) to screen for AD/HD and oppositional defiant disorder (ODD). The instrument also contains the Peer Conflict scale which measures peer aggression, and the Stimulant Side Effects Checklist which monitors medication.
